Acknowledgment of Risk:
I acknowledge that participation in high-intensity Pilates classes at Fuego Pilates (the “Studio”) involves inherent risks, including but not limited to falls, serious and minor physical injury, muscle strain or sprains, cardiovascular stress and death. I understand that the nature of these activities involves rigorous physical exercise, and I assume full responsibility for the risk of any injury or death associated with participation in any activity or class while at, or under direction of Fuego Pilates 2 LLC.
Health and Medical Condition:
I certify that I am in good health and have no medical condition that would prevent my participation in the classes offered by Fuego Pilates. I agree to inform the instructor immediately if I feel any discomfort or pain during a session. I also agree to consult with my physician before starting any new exercise program.
Release of Liability:
In consideration of being allowed to participate in the classes and activities offered by the Studio, I hereby release, waive, discharge, and covenant not to sue Fuego Pilates, its owners, instructors, employees, and agents (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me, or to any property belonging to me, while participating in such activities, or while on or upon the premises where the activities are being conducted.
Indemnification:
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including court costs and attorneys’ fees, that they may incur due to my participation in said activities.
Photo and Video Release:
I grant permission to Fuego Pilates to use my photograph and/or video image in any official publicity materials, including the Studio’s website, social media, and promotional materials.
Acknowledgment of Understanding:
I have read this waiver and release of liability and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
